Malwarebytes earned its place by cleaning machines that a conventional scanner had already declared healthy. The detection approach leans on behaviour and on the specific families that make a desktop miserable rather than on a signature list alone, which is why it finds the browser hijacker, the bundled toolbar, the coin miner and the adware injector that other products walk past as merely unwanted.
The Premium build adds real time protection on top of the on demand scanner, and that is the difference between cleaning up afterwards and not getting hit. Four layers run at once: a web layer that blocks known bad destinations before the page loads, an exploit layer that watches browsers and document readers for the memory tricks that start a drive by, a malware layer that inspects files as they are written, and a ransomware layer that watches for the pattern of mass encryption and stops the process mid run.
Scanning is fast because it is targeted. A threat scan checks memory, startup entries, the registry hives that matter and the folders that malware actually uses, and it finishes in a few minutes on a normal machine. A custom scan will go through an entire drive if you want it to. Quarantine is reversible, so a false positive on a cracked application or a keygen can be restored without reinstalling anything.
The rest is quality of life. Scheduled scans, a browser guard component, an exclusions list that accepts files, folders, applications and websites, a play mode that suspends notifications, and a report view that tells you what was found and where it came from. Running two resident scanners at once causes both to misbehave. Turn off the other one or leave this in on demand mode only.
Add your own tools directory to the exclusions list before the first full scan or you will spend an afternoon restoring things from quarantine.
